Request for Exclusion - Login
If you do not want to be part of the Settlement, you must formally exclude yourself from the Settlement. This is called a Request for Exclusion and is sometimes also called “opting out.” If you opt out, you will not receive a Settlement payment, but you will keep any rights you may have to sue Prudential on your own about the legal issues in this case. You can exclude yourself on an individual basis only; so-called “mass” or “class” opt outs are not allowed.
To start your online Exclusion Request, you must login with your Notice ID and PIN. You can find your Notice ID and PIN at the top of your email or postcard notice. If you are unable to locate your Notice ID and PIN, or if your postcard notice does not include the Notice ID and PIN, please contact the Settlement Administrator by emailing info@PrudentialFinancialDataBreach.com or calling toll-free (833) 360-6875. To request your Notice ID and PIN you will need to provide your full name and mailing address. Alternately, you may download a Request for Exclusion form here and submit it by U.S. mail or email.
To exclude yourself, you must submit an Exclusion Request no later than October 15, 2025.
